Follow their connections.
The clinical record and the lived experience, connected.
The third document brings the first two into relationship: what happened in care, what was happening in life, and what each source says about their connection.
Every piece keeps its thread.
A finding leads back to its source. A patient account remains attributed to the patient. Agreement, conflicting accounts and unresolved questions stay visible. Events happening together do not, by themselves, establish cause.
The result is a packet a person and their clinician can navigate together: a clinical brief, a lived-experience account and an integrated account, with the supporting sources within reach.
Clinical assessment and treatment decisions remain with the clinician.
